• Retirement Category
  Review a summary of your benefits by selecting your retirement category.

• Service Providers
  Select this option from the menu bar at left or from the link above if you are in the Aetna Managed Choice POS plan or the Blue Cross Blue Shield POS plan. You will then see a list of locations. Select your geographic area, then your plan name to access a directory of doctors and other health care providers in your health care plan.

• Online Forms
  Select this option on the menu bar at left or from the link above to download and print any required forms.

• Beneficiary Review or Change
  Log onto the secure Benefits Web site and access your benefits personal home page. Then from the menu bar on the left side of your personal home page, select "Beneficiary Designation." Follow instructions on the beneficiary screens to name or change one or more beneficiaries for plans that require a beneficiary.
