VISION AND HEARING CARE
When you select health care coverage under the Comprehensive
PPO, Standard PPO, Safety Net PPO and POS options, or an HMO
in the Dallas/Fort Worth area, you receive the Vought Aircraft
vision and hearing benefits. Blue Cross Blue Shield administers
these benefits. There is no deductible for these benefits
and no network of providers. You can choose any licensed provider.
Most HMOs outside the Dallas/Fort Worth area provide vision
and hearing coverage. Coverage varies, so check your HMO materials
or call your HMO for details.

VISION CARE
For vision care, the vision benefit pays up to $250 per person
each benefit plan year. Eligible expenses include:
- One exam each benefit plan year
- Two pairs of lenses (including contact lenses) and disposable
contact lenses (up to a one-year supply) each benefit plan
year
- One set of frames every plan year
- Prescription sunglasses, as prescribed by a physician,
that are medically necessary and are not otherwise available
without a prescription.
If you receive routine vision care from an optometrist or
ophthalmologist, your care is covered by the vision benefit.
The medical plan options cover eligible expenses for care
from an ophthalmologist to treat a disease or injury of the
eye. Depending on the option in which you enroll, you may
be required to pay a copayment, deductible and/or coinsurance.
When you visit an optometrist, ophthalmologist, or eyeglass
manufacturer for a vision exam, lenses or frames, you pay
the provider at the time you receive the services. Then, you
can file a claim with Blue Cross Blue Shield for reimbursement
of eligible expenses.

HEARING CARE
The hearing benefit pays up to $500 per ear each benefit
plan year for eligible expenses. This benefit includes one
annual hearing exam, one hearing aid repair, or a new hearing
aid every three benefit plan years. Hearing aid batteries
are not covered.
When you visit a hearing care professional, you pay the provider
when you receive the services. Then, you can file a claim
with Blue Cross Blue Shield for reimbursement of eligible
expenses.
