Prescription Drugs
Special Information For Diabetics
PRESCRIPTION DRUGS
The Comprehensive PPO, Standard PPO, Safety Net PPO and POS
options provide coverage through Caremark for medically necessary
prescription drugs. "Covered" drugs are those that
are necessary for the treatment of a disease or illness and
are widely accepted as effective, appropriate and essential
- based on the recognized standards of the medical community
and as approved by the claims administrator for reimbursement.
In addition, the drug therapy must be prescribed by a licensed
physician and must be in accordance with type, frequency and
duration-of-treatment guidelines of national medical, research
and governmental agencies.
As new drugs become available, the plan administrator decides
whether they are considered an eligible expense under the
Vought Aircraft medical plan.
The medical plan options allow you to purchase medically necessary
prescription drugs from:
- Any Caremark participating pharmacy
- The Caremark Mail Service Program
- Any non-participating pharmacy (covered at 50% of URC
charges).
Your prescription drug coverage depends on where you purchase
your prescriptions, whether you purchase generic or brand
name drugs and whether you file claims electronically or with
a paper claim form.
Prescription drug expenses count toward your medical plan
option's lifetime maximum but do not count toward your out-of
pocket maximum.
Because many chain drug stores are independently owned, some
stores on this list may not be participating pharmacies. To
be sure, call your drug store in advance and ask if it is
a Caremark participating pharmacy. Or, call Caremark directly
at 1-800-407-9435 to find the pharmacy closest to you.

Generic vs. Brand Name Drugs
In general, generic drugs serve the same purpose as brand
name drugs. However, you can save money with a generic prescription.
The brand name manufacturer paid the costs of research, development
and marketing and must recoup these costs through the sales
price. Generic equivalents are not available for all brand
name drugs.
Retail Prescriptions
You can purchase up to a 30-day supply of prescription drugs
at retail pharmacies. To receive the highest benefits available
for retail prescriptions, go to a Caremark participating pharmacy
Caremark Participating Pharmacies
Under the Comprehensive PPO, Standard PPO, Safety Net PPO
and POS options, when you purchase prescriptions at a Caremark
participating pharmacy, your prescriptions are covered at
100% after you pay a copayment. You pay a $5 copayment for
generic prescriptions and a $5 copayment for brand name prescriptions
that cost the same as generic prescriptions. You pay a $15
copayment for brand name prescriptions when no generic equivalent
exists or your state mandates the brand name prescription.
When you purchase a brand name prescription and a generic
equivalent is available, you pay a $25 copayment even if your
physician specifies the brand name drug instead of the generic.
Non-participating Pharmacies
Prescriptions from nonparticipating pharmacies are reimbursed
at 5001o, after you pay the benefit plan year prescription
deductible. The prescription deductible is $25 per person
and $50 per family
When you fill your prescription at a non-participating pharmacy,
you pay the entire amount when you purchase your prescription.
Then, you can file a claim for reimbursement with Caremark.
Claim forms are available from the Vought Benefits Center
or you can download forms from
http://benefits.voughtaircraft.com.
Mail your completed claim and receipts to the address on
the form.
If you are an employee of Vought Aircraft and live outside
the United States, you need to submit a prescription drug
paper claim form. However, these paper claims are reimbursed
as if the prescriptions were purchased at Caremark participating
pharmacies and filed electronically, i.e., 100% (less the
appropriate copayment of $5, $15, or $25).
Mail Order Prescriptions
The Caremark Mail Service Program provides long-term and
maintenance medication at a reasonable price, for eligible
participants residing in the United States. Maintenance medications
are drugs taken on a regular basis for chronic conditions
such as high blood pressure and diabetes. Also, you can purchase
birth control pills through the Mail Service Program.
Mail order prescriptions are available on an in-network basis
only and there is no deductible. With one copayment, you can
purchase up to a 90-day supply of medication. Each mail order
prescription may be refilled three times.
Through the Mail Service Program, you pay a $5 copayment
for generic prescriptions and a $5 copayment for brand name
prescriptions that cost the same as genetic prescriptions.
You pay a $15 copayment for brand name prescriptions when
no generic equivalent exists or your state mandates the brand
name prescription. When you purchase a brand name prescription
and a generic equivalent is available, you pay a $25 copayment
even if your physician specifies the brand name drug instead
of the generic. In some instances, Caremark may contact your
physician to request authorization to substitute one prescription
drug for another.
Important rules about fertility medications
The medical plan options administered by Blue Cross Blue Shield
pay benefits for fertility medications when prescribed to
treat an infertility condition. In some cases, your health
care provider may prescribe fertility medication for a condition
unrelated to fertility. If this is the case, Caremark covers
the fertility medication.

SPECIAL INFORMATION FOR DIABETICS
The prescription drug benefit includes a special provision
for diabetic kits.
You pay only one copayment for a kit of supplies if:
- Your physician lists all of your diabetic supply requirements
on one prescription, and
- You order all of the supplies at the same time through
the Caremark Mail Service Program.
Each time you refill all or part of the kit, you pay another
copayment.
The kit includes these supplies:
- Insulin
- Alcohol wipes
- Diagnostic strips
- Lancets and syringes.
