Updating Your Benefits Records
How Do You Make Changes to Your Benefits Records
Many events require record changes to keep your health care
coverage current. You can get the forms you need to make these
changes from the Vought Benefits Center.
To change your address on your benefits records:
- Complete an address change form (available on the Excel
Web site or from the Vought Benefits Center) and send it
to Payroll, M/S 49-44, Vought Aircraft Industries, P.O.
Box 655907, Dallas, TX 75265-5907.
To change your name on your benefits records:
- Notify your supervisor.
- Complete a name change form (available on the Excel Web site
or from the Vought Benefits Center) and send it to the Security
Department.
- If the name change is the result of a marriage or divorce,
you may wish to make family status changes.
- Indicate the change on your next medical claim form.
How Do You Update Your Marital Status Records
To update your benefits records when you get married:
- Notify the Vought Benefits Center. To change your name,
contact the Security Department. Complete an address change
form if necessary.
- If you want to enroll your spouse (and/or dependent stepchildren)
in Managed Choice POS, complete the appropriate
form from the Download Forms
page within 60 days of your marriage to avoid a delay in
coverage. You must verify eligibility for coverage with
documents that support the change in dependent coverage.
To update your benefits records when you legally separate
or divorce:
- Complete and return the appropriate form available from
the Excel Web site or the Vought Benefits Center within 60
days to delete your former spouse's coverage under Managed
Choice POS. Coverage requirements for dependent
children may also change.
- Notify the Vought Benefits Center within 60 days of the
event if your spouse or dependent children want to purchase
COBRA coverage continuation. In the event of divorce, you
must supply the Vought Benefits Center with a copy of the
final court judgement.
How Do You Update Your Dependent Records
To enroll a new eligible dependent for Managed Choice POS:
- Notify the Vought Benefits Center.
- To avoid a delay in coverage, complete the Add/Change
form along with applicable documentation within 60 days
of the date you acquire the dependent.
What Happens To Coverage When Your Dependent Child Reaches
Age 19
Coverage for dependent children stops when the dependent
reaches age 19. If the child is a full-time student at an
accredited academic institution who depends upon you for support,
you may continue the student's coverage until age 25, as long
as the dependent remains single and qualifies as a full-time
student.
To end coverage, complete the appropriate form available
from the Vought Benefits Center within 60 days of the date
the child becomes ineligible. Contact the Vought Benefits
Center if you wish to enroll the dependent for COBRA coverage
continuation.
