Documenting Changes of Covered Dependents

If you need to add a dependent to your coverage because of a change in life status - marriage, birth or adoption or a child, for example - please complete the Add/Change Form and send it, along with documents that substantiate the change in life status, to the Vought Benefits Center. Examples of documents are a birth certificate, marriage license, adoption papers, court order or evidence of current registration at a college or university for a dependent age 19 to age 25. To add coverage for a stepchild living with you, provide a birth certificate and evidence of place of residence, such as a school record.

In the case of a divorce, to drop coverage on your spouse, you must provide a copy of the final court order. Otherwise, you do not need to provide substantiating documents to drop coverage of a dependent.

The Add/Change Form is available on the Download Forms section of this site, from your site Benefits office or through the Vought Benefits Center. Complete the form and mail it with copies of applicable documents, internally, to Benefits,Dallas, M/S 49L-02; or, externally, to Benefits, M/S 49L-02, Vought Aircraft Industries, P.O. Box 655907, Dallas, TX 75265-5907. Do not send original documents. They will not be returned.
