Identification Cards

Your medical and drug plan identification cards for yourself and each of your covered dependents. The cards contain your group number and other administrative information for you and your health care givers to use in accessing services and filing claims. Present your card to your doctor or other health care provider when you seek health care.

If you find an error on your card or need a replacement, please call your carrier or call the Vought Benefits Center, 1-866-689-5999, as soon as possible. The Center operates Monday through Thursday 8am to 8pm and Friday 8am to 4:30pm, Central time.
